These terms govern your use of the Strata Outpatient website at strataoutpatient.com. They apply to this website only. They are not a treatment agreement, and they do not describe the terms of care — if you become a patient, that is covered by the documents you sign with us.
If you are in crisis, do not use this website.
Call or text 988 to reach the Suicide and Crisis Lifeline, call 911, or go to your nearest emergency room. This site is not monitored for emergencies. Nothing you submit through a form on this site reaches anyone immediately, and no one is watching for urgent messages overnight or on weekends.
Informational purpose only
Everything on this site — the descriptions of our programs, the conditions we treat, any articles we publish, and the biographies of our clinicians — is general information about our practice and about mental health care. It is not medical advice, a diagnosis, or a treatment recommendation, and it is not a substitute for talking to a qualified professional about your own situation.
Do not delay getting care, or stop care you are already receiving, because of something you read here. Mental health conditions present differently in different people, and only an individual assessment can tell you what applies to you.
We write and review this content carefully and update it when things change, but we do not promise that it is complete, current, or free of error at every moment.
No provider–patient relationship
Using this site does not make you our patient. Reading these pages, filling out the contact form, submitting the insurance verification form, calling us with a question, or exchanging email with a member of our team does not by itself create a provider–patient relationship or any obligation on our part to treat you.
That relationship begins only when you have completed our admissions process, we have agreed to accept you into a program, and you have signed our consent to treatment. Until then, nothing anyone at Strata Outpatient tells you through this site is clinical advice about your care, and we are not able to act as your provider.
We may also be unable to accept you, for reasons including clinical fit — some people need a higher level of care than an outpatient practice can safely provide — or capacity. If so, we will tell you and try to point you somewhere appropriate.
What you send us through this site
Our forms are hosted by Jotform on its HIPAA-compliant platform and are encrypted in transit, and we treat what you send with the care described in our Privacy Policy and our Notice of Privacy Practices. Even so, please send us only what we ask for. The forms deliberately request the minimum we need in order to call you back; they are not the place for your clinical history, and you should not use them to tell us anything that needs a response today.
Ordinary email is not a secure channel. If you email us at an address on this site, please keep health details out of it, and we will move the conversation to a secure channel or to the phone.
If you send us an idea, a suggestion, or feedback about the site or our programs, we may use it without owing you anything for it. That does not apply to information about you or your health, which is governed by the two documents linked above.
Using the site
You are welcome to read, print, and share these pages for your own personal, non-commercial use, including to help someone decide whether to seek care.
Please do not:
- attempt to gain access to any part of the site, or any system or account connected to it, that is not open to the public;
- interfere with the operation of the site, or with anyone else’s use of it, including by submitting forms automatically or in bulk;
- use the site to send anything unlawful, threatening, or deliberately false, or to impersonate someone else;
- collect information about our clinicians or other visitors from the site by automated means; or
- copy substantial parts of the site, or republish them, as though they were your own.
Our content and our name
The text, photographs, design, and layout of this site belong to Strata Behavioral Healthcare™ or are used by us with permission, and are protected by copyright. Strata Behavioral Healthcare and Strata Outpatient are our marks and identify our services. Nothing on this site grants you a license to use them.
Where we display someone else’s mark — an insurance carrier we work with, or a directory that lists us — it belongs to that company, and its appearance here does not mean they endorse us beyond the relationship it describes.
Links and services from other companies
Parts of this site are provided by other companies, and some pages link out to them — our forms, the map of our location, our patient portal, event registration, and professional directories. When you use one, you are dealing with that company under its own terms and its own privacy policy, which we do not control. Our Privacy Policy lists which services are involved and links to each of their policies.
A link from this site is a pointer, not a recommendation of everything you will find at the other end.
Availability
We try to keep the site up and working, but we do not guarantee it. It may be unavailable for maintenance, because something has broken, or for reasons outside our control, and we may change or remove any part of it at any time. If you need to reach us and the site is not working, call (858) 567-4246.
Disclaimer and limits on our liability
This site is provided as it is. To the fullest extent the law allows, we disclaim all warranties about it, express or implied, including any implied warranty that it is fit for a particular purpose, and we do not warrant that it will be uninterrupted, secure, or error-free.
To the fullest extent the law allows, Strata Behavioral Healthcare, Strata Outpatient, and our owners, clinicians, and staff are not liable for any indirect, incidental, or consequential loss arising out of your use of this website or your reliance on information published on it.
This does not limit our responsibility for the care we actually provide. Nothing in these terms limits any right you have as a patient, or any duty we owe you as your provider, and nothing here affects any liability that cannot be limited under California law.
Changes to these terms
If we change these terms we will replace this page and change the effective date at the top. The version posted here is the one that applies.
Governing law
These terms are governed by the laws of the State of California, without regard to its conflict-of-laws rules. Any dispute about this website will be brought in the state or federal courts located in San Diego County, California.
If any part of these terms turns out to be unenforceable, the rest still applies.
